Congratulations, Harleyville Elementary School! The Dolphins earned an Average rating on the South Carolina School Report Card and were the district’s biggest mover, increasing their rating by 17 points.

Click INCOMPLETE FORMS for each student and complete all required forms
Sign your full name in the parent signature field on each form and click SUBMIT FORM
Before you sit down to register, have this information handy:

Basic medical history and health information
Insurance company and policy number
Doctor, dentist, and specialist contact information
Hospital preference and contact information
Once all forms are submitted, you will see a "Forms Finished" confirmation. Need to add another student? Just click MY STUDENTS and repeat the process.
